It is important you understand that certain information which you provide on your application, will be investigated as required by section 391.23 of the Federal Motor Carrier Safety Regulations, and that previous employers may be contacted to verify this information. There are 4 major sections that require applicant information. It is highly recommended that you have this information available and readily at hand prior to proceeding.

• Personal Information - Contact and other personal information.
• Employment History - Minimum 3 years of previous employer information, plus an additional 7 years if any employment during that period involved driving a commercial motor vehicle
• Driving Information - Accident & traffic violations
• Experience & Qualifications - License information & driving experience
